Abstract

Chimeric antigen receptor (CAR) T-cell therapy targeting CD19 or B cell maturation antigen has revolutionized the treatment of patients with relapsed lymphoma, acute lymphoblastic leukemia, and multiple myeloma. However, this therapy is associated with both short- and long-term immune-mediated toxicities. Among them, neurological complications represent a distinct and clinically significant category, with a wide range of presentations. In this paper, we describe the different types of immune effector cell–associated neurotoxicity syndrome (ICANS) and non-ICANS neurological toxicities, including their clinical features, recommended diagnostic evaluations, underlying mechanisms, and available treatment approaches.
