Abstract

Introduction: Leukapheresis is the starting point of cellular therapies such as chimeric antigen receptor T cells (CAR-T cells) and donor lymphocyte infusion. Methods: After a review of the literature, we provided guidance on how to evaluate candidates for leukapheresis and how to optimize the safety and efficacy of the procedure. Results: Clinical and laboratory assessment is warranted prior to leukapheresis. T cell fitness is impacted by previous treatments, ageing, and tumor burden. The most common adverse reaction is hypocalcemia. Conclusion: Leukapheresis is a safe and effective procedure. Technical and clinical aspects must be observed to preserve T cell fitness, ensure manufacturing success, and guarantee patient safety and clinical efficacy. The current challenge is to standardize guidelines for pre-leukapheresis requirements and collection yield improvement.
